Sinopse: A história gira em torno de Lucas Scott (Chad Michael Murray), um jovem de 16 anos que sonha em ser jogador de basquete. Vive com a mãe, Karen Roe (Moira Kelly), e é meio-irmão de Nathan Scott (James Lafferty). Seu pai, Dan (Paul Johansson), nunca o aceitou como filho, deixando-o sob criação total da mãe. Ao ser convidado para se juntar aos Ravens, time de basquete da Tree Hill High, escola onde estuda, Lucas tem de lidar com a pressão do pai e o temperamento forte do irmão Nathan. Além disso, ele se apaixona por Peyton Sawyer (Hilarie Burton), namorada do irmão e cheerleader do time. Para piorar, Brooke Davis(Sophia Bush), melhor amiga de Peyton, se apaixona pelo jovem. Nathan usa Haley (Bethany Joy Galeotti), melhor amiga de Lucas, como alvo para atingir o garoto. Porém, seus planos dão errado, e ele acaba se apaixonando pela moça. Lutando pela aceitação do pai e enfrentando brigas com o irmão, Lucas tem de equilibrar sua vida emocional com o basquete.

Ao longo das temporadas, One Tree Hill ganha alguns saltos no tempo e muda seu foco. A série deixa de ser considerada teenna quinta temporada, quando a temática sobre o basquete e problemas adolescentes diminuem. A medida que os personagens crecem e o tempo passa, a storyline baseia-se em problemas adultos: Emprego, gravidez e relacionamentos frustrados, além de explorar temas polêmicos sobre anorexia, abandono, adoção, drogas e bebida.
